YES NO Please submit a copy of your 2016 federal Form 1040, 1040A or 1040EZ along with your exemption request. Did you receive TANF (Temporary Assistance to Needy Families) or SNAP (Supplemental Nutrition Assistance Program) benefits? YES NO Please provide a copy of your TANF or SNAP award letter along with your exemption request. Does your child (or children) receive free lunch?
